Our Care Management System Integrates High Tech and a Soft Touch

Every client’s care management plan is input into Advocare’s “Virtual Medical Chart” in our secure, web-based case management system, called Advocate Anywhere.  This is a unique & proprietary program that has helped Advocare to set a new standard in follow through of the care management plan. Information on the care management plan for each client is entered in our system, starting right from our first assessment. Case notes are updated upon every interaction with our client. When there is an important update such as new physicians, medications, or doctors visits, our Care Manager updates the Virtual Medical Chart, and communicates any updates of the care management plan to designated family members and medical providers.
