- Advise. After the full assessment is completed you can expect a customized care plan based on our findings. This three to five page document will go over everything we went over during the initial assessment. - Advocate. The third and final step is for our aging life care manager to arrange for the services you and your family has selected. We can also answer any questions you may have about the different options you have available. Communication between you and our aging life care manager will best keep you involved in selecting the right plan for your parent or loved one.
